Step by Step
Get Assessed
In-Home Visit: A nurse will visit you at home to determine if you are eligible for the CDS program. You'll find out how many weekly hours of care you can receive.
Choose Caregiver
Choose a caregiver that's right for you. Your caregiver CAN be a family member or Friend. (Legal Spouses are not eligible to be a caregiver)
You get Care, They Get Paid
You will receive on-going care from someone you know and trust. Your caregiver will receive hourly compensation for the services that they provide.
Get Assessed
Contact Us
Call us at (314) 305-8294. Our intake representative will help you get started on obtaining authorization from the patient's Medicaid insurance.
Intake Forms
We will provide you with the forms (Nurse Assessement and Medical Forms) to be completed by the patients doctor.
In-House Visit
One of our field specialist will conduct an in-home visit and orientation with the patient and Caregiver, ensuring all parties involved understand the process and responsibilities.
Hiring a Caregiver
Choose your Caregiver
Choose a caregiver that's right for you. Your caregiver CAN be a family member or Friend. (Legal Spouses are not eligible to be a caregiver). Caregiver must be at least 18 years of age.
Application
Your Caregiver will complete an employment Application (Provided by our Intake Specialist)
FCSR Screening
Our intake Specialist will complete the FCSR background screening.
Employee Portal Setup
A member of our Welcome Team will welcome the patient and Caregiver into our Program, Assign Username and Password for our Web-based time tracking system and Train users on how to use.
You're All Set!
You will now receive the Care you deserve from someone you know and Trust and Your Caregiver will begin receiving hourly compensation for providing their services.
